Student Procedure For Medical/Compassionate Withdrawal
The following procedure applies to students who must withdraw from classes for medical or other significant, meaningful personal reasons. To receive a medical/compassionate withdrawal, you must complete the following steps:
- Provide appropriate documentation.
For a medical withdrawal, provide a letter from your attending health care provider that specifies the date of onset of illness, the dates you were under professional care, the general nature of your medical condition and why/how it prevented you from completing your course work, the date of your anticipated return to school, and the last date you were able to attend class. Application for less than a complete withdrawal must be especially well documented to justify the selective nature of the medical/compassionate withdrawal request. The letter must be typed on the health care provider's letterhead stationery and submitted in a sealed envelope.
For a compassionate withdrawal, provide the documentation that is appropriate to your circumstances.
NOTE: The documentation that you provide will be verified. Please give the provider of your documentation written permission to discuss your case with the college medical withdrawal designee who may contact him or her for more information or clarification.
- Obtain and complete a Request for Documented Medical/Compassionate Withdrawal form from the Office of Undergraduate Programs, Foundation suite 110 or from this Web site. A request for less than a complete withdrawal will require a personal letter of explanation.
- Submit these materials to the College of Liberal Arts and Sciences, Office of Undergraduate Programs, P.O. Box 876605, Foundation suite 110, Tempe, AZ 85287-6605.
- The college medical withdrawal designee will determine whether
a medical/compassionate withdrawal will be granted in your case.
You will be notified of the decision by mail.
Medical and compassionate withdrawals are not obligations of the university/colleges. Each college establishes its own review procedures.
NOTE: You may be subject to registration restrictions when you return to school.
